ID Option Donors - The donor agrees that their identity can be released later when the child turns 18

- Can make a 6-12 month commitment - Live within 40 miles of the Berkeley office - Able to work legally in the United States - Can provide a family medical history - Sperm count must meet minimum requirements - Must abstain from ejaculating for 3 days (72 hours) before each sample - More than 4 days of abstinence is not recommended - FDA-required regular blood and urine testing for STIs - Physical exams - Expanded carrier screening (hundreds of genes) with genetic counselor review - Must provide a personal narrative and baby photo - Welcome men of all ethnicities and backgrounds

- Step 1: Complete an Online Application - Step 2: Determine Your Eligibility - Step 3: Screening & Testing (STI testing, genetic testing, physical exam, family medical history review with genetic counselor, personal narrative, baby photo, interviews, education sessions) - Step 4: Contract Signing - Step 5: Exit STI Testing (6 months after final donation, paid $700)
